It is with Tom's 5D and 24mm tilt/shift lens tilted backward to create a very shallow depth of field.
I know that Robert will point out that you can now do this is Photoshop CS 6.
One of my favorite landscape photographers is the Brit, David Noton. I have a couple of his books I enjoy not only for his photography skills - and technical suggestions - but his witty use of the written word. He describes using Canon's 24mm TS even for shots not requiring tilt/shift, because he considers the lens to be Canon's sharpest.
